'Journey to wellbeing — huarahi ora' is a guide for primary care teams supporting people with long term conditions. The guide is designed to enable clinicians to better support people with their day to day management of their conditions.

The guide is a shift away from what was known as 'self-management support' (SMS), to an approach that is more:

  • whānau inclusive
  • culturally safe
  • community-designed.

The resource helps clinicians understand this new approach to helping patients manage their long term conditions. It includes:

  • a self-assessment of your knowledge of the new approach
  • how to work towards being a leader in primary care in the management of long term conditions
  • ways to build team capability where it is needed.

Seven principles

'Journey to wellbeing — huarahi ora' is based around waka, which represents a journey. It includes 7 principles to consider when designing and developing programmes and initiatives for people who have long term health conditions.

  1. Whānau centred — the whānau voice is central and they are involved in decisions, actions and practices. People are experts in their own wellbeing.
  2. Co-designed — partnering with community to meet people's needs and priorities. Prioritise community relationships and build capability.
  3. Culturally safe — respect for cultural practices, culturally safe content, diverse and inclusive workforce.
  4. Meaningful — holistic and innovative, outcomes focused, responsive and supports behaviour change.
  5. Accessible — welcoming, integrated, safe and reliable, supported participation.
  6. Skilfully facilitated — workforce development and support, lived experience, mentoring.
  7. Sustainable — continuity, resourced, leadership, connected and evaluated.
